- Research and select the lab tests or wellness packages that meet your needs on the Ulta Lab Tests website.
- Once you’ve chosen your desired tests or packages, add them to your cart and proceed to the secure online checkout.
- For new users, you will be prompted to create an Ulta Lab Tests account. Returning users can simply log in to their existing accounts.
- Upon successful purchase, you will receive an automated confirmation email receipt.
- Also, you’ll receive a lab requisition order that needs to be printed out for your upcoming lab visit.
Already Have a Lab Order?
- If you already have a lab requisition, you can send it directly to us, or have your doctor’s office securely fax it over. Once we receive it, our team will reach out promptly to schedule your at-home or on-site lab draw.
- Safely upload your existing lab order directly to our SimplePractice EHR. Your information is encrypted and handled confidentially.
